SVgl1Y�j� <br /> t ff <br /> tr n <br /> • x <br /> •° CITY OF SUNNY ISLES BEACH AGREEMENT <br /> ‘ o.��': WITH A & A DRAINAGE & VAC SERVICES, INC. <br /> C/r 0I fUM 0 <br /> CONTRACT NO: C1011-053\ <br /> THIS AGREEMENT (the "Agreement"), is entered into as of k14-1 c IS* , 2011 <br /> (the "Effective Date"), by and between City of Sunny Isles Beach ("CITY"), a municipal <br /> corporation located at 18070 Collins Avenue, Sunny Isles Beach, Florida 33160 and A & A <br /> DRAINAGE & VAC SERVICES, INC. a Florida corporation, authorized to do business in the <br /> State of Florida (hereinafter referred to as the "Contractor"), whose Federal I.D.# is <br /> WITNESSETH : <br /> WHEREAS, the City is in need of a contractor to clean storm water catch basins and <br /> jetting of sewer lines throughout the City limits ("Services"), as more particularly described in <br /> Invitation to Bid No. 11-04-03, which is incorporated herein by reference; and <br /> WHEREAS, the Contractor submitted a bid and was deemed the lowest responsible, <br /> responsive bidder in response to Invitation to Bid No. 11-04-03 for the Services, which is <br /> attached hereto and incorporated herein by reference as Exhibit"A"; <br /> WHEREAS, the Contractor is qualified, willing and able to provide the Services on the <br /> terms and conditions set forth herein; and <br /> WHEREAS, the City desires to enter into this Agreement with Contractor to provide the <br /> Services in a total annual amount not to exceed Forty Thousand Three Hundred Twenty Dollars <br /> ($40,320.00). <br /> NOW THEREFORE, in consideration of the foregoing and for the mutual covenants, <br /> representations and warranties and other good and valuable consideration, the receipt and <br /> adequacy of which is hereby acknowledged,the parties agree as follows: <br /> 1. TERM. The term of this Agreement shall commence upon the issuance of a notice to <br /> proceed from the City Manager or his designee and shall remain in effect for one year thereafter. <br /> 2. OPTIONS TO RENEW. Prior to, or upon completion of the initial one year term, the <br /> City shall have the option to renew this Agreement for up to four (4) consecutive one (1) year <br /> renewal terms. <br /> 3. COMPENSATION. During the initial term of this Agreement, the Contractor agrees to <br /> provide the Services in an amount not to exceed Forty Thousand Three Hundred Twenty Dollars <br /> ($40,320.00). Prior to, or upon completion of the initial term, the City may consider an <br /> adjustment to the price for any renewal term based on the procedures set forth in Section 2.4 of <br /> Invitation to Bid No. 11-04-03. <br /> Attachment "B" €Y_ <br /> a` �) <br />
