STATE OF FLORIDA DEPARTMENT OF TRANSPORTATION S75.02G43 <br />CONTRACTOR MAINTENANCE <br />FIELD PERFORMANCE REPORT W14 <br />Contractor: Envirowaste Services Group, Inc. <br />Contract: E2V99 <br />Evaluation Date: May 21, 2018 <br />Type of Evaluation: 0 Periodic (j) Final <br />Pursuit and Timely Completion of Work <br />Performance <br />The Contractor performed all work in an effective and expedient manner. The Department's <br />Excellent <br />expectations regarding timely pursuit and completion of the work were often exceeded. <br />Except for a few occasions, the Contractor completed all of the work within authorized timeframes. The <br />Satisfactory <br />Contractor consistently demonstrated sufficient efforts to complete work in a timely fashion. The <br />Satisfactory <br />Department's expectations regarding timely pursuit and completion of the work were usually met and <br />0 <br />sometimes exceeded. <br />The Contractor occasionally completed the work in a timely manner, but efforts were consistently less <br />Unsatisfactory <br />than adequate. Department involvement was required on more than one occasion to prompt the <br />O <br />Contractor to complete the work, <br />0 <br />The Contractor failed to complete the work in a timely fashion. On several occasions the Contractor <br />Poor <br />failed to begin work in a timely fashion, and the Contractor made little effort to correct deficiencies. <br />Poor <br />Substantial Department involvement was required to prompt completion of the work, including written <br />0 <br />correspondence advising the Contractor of potential default. Expectations were not met. <br />Notes & Comments <br />The Contractor accomplished all work in an expedient manner as additional crews were utilized to ensure the job was <br />completed in accordance with the contract. It was the expertise and organizational skills of the Contractor that made them <br />realize extra personnel were needed The Department commended the Contractor for being self-sufficient and cognizant <br />of the time frames in the contract. <br />Maintenance of Traffic (MOT) & Safety Operations <br />Performance <br />MOT setups and compliance were proper with no correctable complaints from Department personnel <br />and/or traveling public. No incidents or injuries occurred within work zones due to improper MOT. <br />Excellent <br />Qualified MOT personnel were present at all times. Expectations were exceeded, Contractor often <br />went above and beyond FDOT Design Standard requirements to ensure safety for both work crews and <br />travelin ublic. <br />MOT setups and compliance were mostlyproper with few correctable complaints from Department <br />and/or traveling No incidents or injuries to improper <br />Satisfactory <br />personnel public. occurred within work zones due <br />0 <br />MOT. Qualified MOT personnel were present. Expectations were met and exceededoccasionally. <br />MOT setups and compliance were at times achieved, but there were several complaints and/or <br />Unsatisfactory <br />incidents of non-compliance. No major incidents or injuries occurred within work zones due to improper <br />MOT. Qualified MOT personnel were usually present. Some involvement from Department personnel <br />0 <br />was required on a few occasions. Expectations were not consistently met. <br />Proper MOT deployment and overall compliance was lacking. Numerous correctable complaints from <br />Poor <br />Department personnel and/or traveling public were noted. An incident with injuries may have occurred <br />within the work zone due to improper MOT. Qualified MOT in <br />0 <br />personnel were seldom on site or close <br />proximity to the work site. Expectations were not met. <br />N/A <br />N/A (To be used when MOT was not necessary or included in the contract). <br />0 <br />Notes & Comments <br />No involvement was needed from the Department as all MOT setups were in compliance with the Design Standards. All <br />lane closures were setup properly and the devices were in accordance with the Approved Product Ust (APL). No <br />complaints were received from the traveling public as safety of the work zone was a major concern of the Contractor. <br />
