Section 4 Response Format 4.0 FORMAT "Y or Sot, �-r. Submittals shall be submitted in duplicate. Submit one (1) original, four (4) copies and one (1) electronic copy of the submittal on USB Drive or CD. All required signatures shall be manual, in blue ink of an authorized representative who has the legal authority to bind the CONTRACTOR in contractual obligations. Each page of the bid should state the name of the CONTRACTOR, the bid number, and the page number. The City reserves the right to request additional data or material to support bid. All material submitted in response to the RFP will become the property of the City. LABEL EACH SECTION AS NUMBERED The proposal must be in the following format at the time of submittal: 1. Company Information In response to this Proposal, all Contractors must provide the following: ■ Name of Agency/Company (including any "Doing Business As" names) • Company Location(s) ■ Internet Web Site Address (if any) ■ Details of Entity Business Structure (Corporation, Partnership, LLC) • Date Founded • Office address and telephone number, email address ■ List of any outstanding litigation that would threaten the viability of the firm or the performance of this contract ■ Proof of insurance ■ Proposer must submit a copy of Florida Division of Corporations Sunbiz report with your company registered as active. ■ W9 2. Qualifications Proposer's relevant experience, qualifications and past performance ■ An explanation of why the Contractor is the best qualified to perform the contract, include General Contractor's license. 3. Staffing Relevant experience and qualifications of key personnel, including key personnel of subcontractors, that will be assigned to this project and experience and qualifications of subcontractors • The size and experience of the company staff pool from which staff assigned to the management contract can be drawn The composition of the staff team that will be assigned to the contract, their function in the company. • The name of the person who will be responsible for the coordination of work.
