A copy of firm’s Articles of Incorporation listing the approved signatories of the corporation; <br />b. A copy of a resolution listing the members of staff as authorized signatories for the firm; <br />and <br />c. A letter from a corporate officer listing the members of staff that are authorized signatories <br />for the firm. <br /> <br /> H. Variations/Exceptions: Provide a list of services which are not included in the firm’s proposal to <br /> the required services as outlined in the Scope of Services along with any exceptions or variations <br /> to any section of the proposal and explanation. <br /> <br />I. Litigation Statement: A statement that no litigation or regulatory action has been filed against your <br /> firm(s) in the last five (5) years shall be included in the proposal. If an action has been filed <br /> against your firm(s) within the last five (5) years, state and describe the litigation or regulatory <br /> action filed against your firm and identify the court or agency before which the action was <br /> instituted, the applicable case or file number, and the status or disposition for such reported <br /> action. Describe all litigation (include the court and location) of any kind involving proposer or <br /> proposer’s team members within the last five ye ars. Indicate if your firm has been debarred or <br /> suspended from bidding or proposing on a procurement project by any government entity during <br /> the last five (5) years. Litigation history will not be a disqualifier to the respondent, such history <br /> will be evaluated by the City. <br /> <br />J. Forms and Attachments <br /> <br />a. Enclosed Forms; <br />b. Respondent must complete, sign as required, and submit the Addenda and all forms in the <br />following Section 6 as part of its submittal; <br />c.
