ADDENDUM # 2 <br />DATE: December 13, 2019 <br />TO: ALL PROPOSERS <br />FROM: Purchasing Agent <br />TITLE: RFQ 19-11-01 Professional Engineering and Architectural Services for Pedestrian Bridge with Park <br />Elements across A1A at 174TH ST. CCNA <br />SUBJECT: Addendum No. 2 <br />BID SUBMITTAL DEADLINE: Friday, December 20, 2019 at 2:30 PM Monday, January 13, 2020 at 2:30 <br />PM <br /> <br />Please attach this addendum to the documents in your possession and include the Acknowledgement <br />of Addenda form with your response incorporated in the RFQ. This addendum is and does become a <br />part of the RFQ 19-11-01 Professional Engineering and Architectural Services for Pedestrian Bridge <br />with Park Elements Across A1A at 174TH ST. CCNA. <br /> <br />Questions & Answers: <br />Q1. NOTE on Page 11: The City of Sunny Isles, Contract Number and Title must be on each Certificate of <br />Insurance (COI). The City of Sunny Isles Beach must be shown as additional insured. Our insurance <br />company doesn’t typically issue COIs containing this specific information until contract negotiations. <br />Would the City accept a letter from our insurance company stating that our firm has the ability to comply <br />100% with your specific request or a sample insurance certificate at this time in lieu of the requirement <br />stated above? <br /> <br />If so, we would provide a COI that names the City as additional insured and includes The City of Sunny <br />Isles, Contract Number and Title during contract negotiations. <br />A1. Proof of insurance will suffice prior to contract award. <br /> <br />Q2. Page 19, 2nd paragraph: Firm shall be pre-qualified with FDOT in the following classes of work by <br />the by the time the agreement <br />is fully executed: <br />Work Group 3. Highway Design Roadway <br />Work Group 4. Highway Design – Bridges <br />Work Group 5. Bridge Inspection <br />Work Group 6. Traffic Engineering & Operations Studies <br />Work Group 7. Traffic Operations Design <br />Work Group 8. Surveying & Mapping <br />Work Group 9. Soil Exploration, Material Testing and Foundations <br />Work Group 14. Architect <br />Work Group 15. Landscape Architect <br /> <br />Does the prime “firm” have to be prequalified in ALL work groups themselves? <br />Or can the proposed “team” cover ALL of these work groups? <br />A2. The answer is no the prime firm does not need to be prequalified in all work groups themselves. <br />Sub-consultants can cover work groups however, based on the FHWA 1273 form, the prime contractor <br />shall perform with its own organization contract work amounting to not less than 30 percent (or a <br />greater percentage if specified elsewhere in the contract) of the total original contract price. <br /> <br />Q3. Calvin, Giordano & Associates asked at the RFQ conference if they may submit a response to the <br />RFQ, although they prepared the 174th Street Pedestrian Overpass Planning and feasibility studies that <br />was attached to the RFQ. <br />A3. Yes, Calvin, Giordano & Associates is permitted to submit a sealed response to the RFQ for review. <br /> <br />Q4. Will the due date of December 20 be changed? <br />A4. The RFQ opening has been extended to Monday, January 13, 2020. <br /> <br />Q5. On page 12 of the RFQ it specifies that “The sub-consultant is subject to the same contractual <br />conditions as is the Respondent”, we understand under the contractual conditions but not on the $10 <br />million dollar Professional Liability Insurance requirement, correct? <br /> <br /> Do sub-consultants also have to carry insurance? <br />A5. It has been confirmed that only the awarded firm has to maintain insurance. The City is not in <br />privity of contract with any sub-consultant. If an issue arises, the City will look to the Consultant’s <br />insurance. <br /> <br />Addition to Section 2.9: <br /> <br />The insurance policy must remain active from the time of contract award, to 5 years after <br />project completion. <br />101 <br />City of Sunny Isles | Request for Qualifications No. 19-11-01
