Forms and Attachments <br /> <br />All affidavits and forms have been acknowledged and completed by an official having legal authorization to <br />contractually bind the Proposer. ECS Group will submit a current certificate of insurance, naming the City of Sunny <br />Isles Beach as an additional insured and listed as such on the insurance certificate when selected. Please see <br />the below forms to meet the following requirements of the City’s RFQ: <br /> <br />a. Enclosed Forms; <br />b. Respondent must complete, sign as required, and submit the Addenda and all forms in the following <br />Section 6 as part of its submittal; <br />c. Firm’s Current Certificate(s) of Insurance; and <br />d. Respondent shall download required electronic forms on the City’s page for this procurement item and/or <br />on DemandStar.com <br />e.
