Russo, City Manager <br />FROM: <br />Susan Simpson, Assistant City Manager <br />DATE: <br />10/15/2020 <br />RE: <br />Resolution Ratifying Change Orders #2, #3, and #4 with <br />Temple, Inc. for Crosswalk Enhancements <br />RECOMMENDATION: <br />Staff is recommending the City Commission approve the attached <br />Resolution. <br />REASONS: <br />The City of Sunny Isles Beach wishes to ratify Changes Orders #2 and <br />#3 with Temple, Inc. providing and installing solar wireless <br />Rectangular Flashing Beacon Assembly (RRFB) products. Change <br />Orders #2 and #3 were both extension of time, with no monetary <br />value. Due to the delay in releasing Contractor to proceed with <br />contract work, additional time was allowed to complete the project. <br />The City now wishes to approve Change Order #4 to the Agreement to <br />include a solar wireless push button to the North Bay Road Pedestrian <br />Bridge north cross walk. With the contract completion date November <br />12, 2020. <br />The total cost of this additional service, is in an amount not to exceed <br />$4,315.00 <br />FUNDING SOURCE: <br />300-5-5410-465000-99008 <br />ATTACHMENTS: <br />Description <br />Resolution <br />Change Order#2 <br />Change Order #3 <br />Change Order #4 <br />Item Number: 10.1). <br />
