Our Proposal is based on a monthly inspection . of all equipment <br />listed below subject to the guidelines set forth- after. <br />Assumptions and Qualifications <br />(12) inspections will be made under this agreement for a period of I year. Payment to be made <br />after each inspection is completed. <br />A follow up report will be provided with all the readings and service description done W each <br />equipment following the inspection: <br />Scope of Work <br />See Attached Maintenance Checklist and.Sto.pe of Work <br />I <br />Scirvice calls will be performed Monday thru Friday at a reduced rate of $ 95.00 per man hour. <br />Emergency service will be performed at reduced rate times 1.5 <br />Contract customers to have perfered emergency service above others <br />Attached PSW equipment list to he serviced <br />Page 1 of 3 <br />
