Section 5 <br />Submittal Format <br />5.0 FORMAT <br />`11Y of Soo "", <br />Submittals shall be submitted in duplicate. Submit one (1) original, four (4) copies and one (1) electronic <br />copy of the submittal on USB Drive or CD. All required signatures shall be manual, in blue ink of an <br />authorized representative who has the legal authority to bind the CONTRACTOR in contractual obligations. <br />Each page of the RFP should state the name of the CONTRACTOR, the bid number, and the page number. <br />The City reserves the right to request additional data or material to support bid. Company Information, in response to this Proposal, all Proposers must provide the following: <br />■ Name of Agency/Company (including any "Doing Business As" names) <br />■ Company Locations <br />■ Internet Web Site Address (if any) <br />■ Details of Entity Business Structure (Corporation, Partnership, LLC) <br />■ Current IRS Form W-9 <br />■ Date Founded <br />■ Home office address and telephone number, and local address and phone number <br />■ List of any outstanding litigation that would threaten the viability of the firm or the <br />performance of this contract <br />■ Proof of insurance <br />■ Email address <br />■ Point of Contact <br />■ Identify Group applying for <br />2. Qualifications <br />Proposer's relevant experience, qualifications and past performance <br />■ An explanation of why the Vendor is the best qualified to perform the contract and <br />demonstrate its qualifications based on the specs. List all equipment the Contractor <br />own/lease for this service. <br />3. Staffing <br />Relevant experience and qualifications of key personnel, including key personnel of <br />subcontractors that will be assigned to this project and experience and qualifications of <br />subcontractors; and the size and experience of the company staff pool from which staff assigned <br />to the management contract can be drawn.
