FOURTH AMENDMENT TO THE AGREEMENT BETWEEN <br />THE CITY OF SUNNY ISLES BEACH AND <br />ACTION LABOR MANAGEMENT, LLC D/B/A <br />STAFFING CONNECTION CONTRACT NO. 2019 - 4775 <br />This Fourth Amendment to the Agreement between the CITY OF SUNNY ISLES BEACH <br />("City') and ACTION LABOR MANAGEMENT, LLC D/B/A STAFFING CONNECTION, <br />("'Contractor") executed this day of ��_'VUAWECV- _, 2021, is made a part of the original <br />Agreement between the parties dated September 15, 2016, Contract No. 4775-1516-118, and all <br />amendments thereto ("the Agreement"), between the City and Contractor attached hereto as Attachment <br />"A" whose. Federal Identification # is 65-1038617, The City and Contractor hereby agree as follows - <br />1. AMENDING TERM. The patties hereby amend Section 3 of the original Agreement to <br />grant the City the option to renew for an additional four (4) optional one (1) year renewal terms. <br />2. OPTION TO RENEW. Pursuant to Section 1, above, the City hereby elects to exercise its fourth <br />and final option to renew the Agreement until September 30, 2022, <br />3. EFFECTIVE DATE OF TINS FOURTH AMENDMENT. The Effective Date of this Fourth <br />Amendment shall be August 23, 2021. <br />4, SERVICES. The parties further wish to amend and restate Section 2.1 of the Agreement, <br />"Services", as follows: <br />2.1 Contractor agrees to furnish thirteen (13) individuals with the proper skills and <br />certifications necessary to perform <br />1, - , the functions of school crossing guards ("Crossing Guards"), <br />and one (1) individual with the proper skills and certifications necessary to perform the functions <br />of school crossing guard supervisor ("Crossing Guard Supervisor"), for each school day at the <br />following nine (9) designated Crossing Guard locations located within the corporate limits of <br />the City: <br />M <br />Crossing Location <br /># of Guards Required <br />_ <br />1 <br />183' St Collins Ave <br />2 <br />2 <br />178th St Collins Ave <br />2 <br />3 <br />183rd St / Atlantic Blvd. <br />2 <br />4 <br />182nd St 1 N. BayItd <br />I <br />However, the numbers included in this Section are estimates, and may be revised pursuant to <br />Paragraph 3.6 of Invitation to Bid No. 16-04-05 . <br />5. ADDITIONAL COMPENSATION. As of the Effective Date of this Fourth Amendment, the <br />City agrees to pay Contractor in the amount of Twenty -One Dollars ($21.00) hourly rate per Crossing <br />Guard, and the Crossing Guard Supervisor. <br />Action Labor Management, LLC.
