City of Sunny Isles Beach |Request for Qualifications No. 21-11-01 25 <br />G.Corporate Standing and Authorized Signatory <br />Respondent must demonstrate that the company is in good standing and that the person signing <br />this submittal is an Authorized Signatory on behalf of the respondent to sign bids, proposals, <br />negotiate and/or sign contracts, agreements, amendments and related documents to which the <br />respondent will be duly bound. The respondent must provide a copy of the State Certificate of good <br />standing listing the officers of the company. If the signatory is not one of the officers listed on the <br />State Certificate, the respondent must provide one of the following forms of evidence of Signatory <br />Authority with its response: <br />a.A copy of firm’s Articles of Incorporation listing the approved signatories of the corporation; <br />b.A copy of a resolution listing the members of staff as authorized signatories for the firm; <br />and <br />c.A letter from a corporate officer listing the members of staff that are authorized signatories <br />for the firm. <br />H.Forms and Attachments <br />a.Enclosed Forms; <br />b.Respondent must complete, sign as required, and submit the Addenda and all forms in the <br />following Section 6 as part of its submittal; <br />c.Firm’s Current Certificate(s) of Insurance; and <br />d.Respondent shall download required electronic forms on the City’s page for this <br />procurement item and/or on DemandStar.com <br />e.Respondents must be properly registered to practice their profession and licensed to <br />engage in contracting in the State of Florida at the time of Proposal submission. The <br />Respondent shall submit copies of their requisite Licenses. <br />END OF SECTION
