Notification regarding registration in System for Award Management <br />(SAM) database. <br />(4) Amendments to Federal Supply Schedule Price Lists must include on the cover page the same <br />information as the basic document plus the title "Supplement No. (sequentially numbered)" and <br />the effective date(s) of such supplements. <br />(5) Accuracy of information and computation of prices is the responsibility of the Contractor. <br />(6) Inclusion of incorrect information will cause the Contractor to resubmit/correct the Federal <br />Supply Schedule Price List, and may constitute sufficient cause for Termination, applying the <br />provisions of 552.212-4 , Contract Terms and Conditions (paragraph (m), Termination for <br />Cause), and application of any other remedies as provided by law including monetary recovery. <br />Page: 117 of 141 <br />
