The City shall not be responsible for <br />any expense incurred by the Contractor. The City shall have no duty to withhold any Federal <br />income taxes or pay Social Security services and that such obligations shall be that of the <br />Contractor, other than those set forth in this Agreement. Contractor shall furnish its own <br />transportation, office and other supplies as it determines necessary in carrying out its duties under <br />this Agreement. <br />7. CRIMINAL BACKGROUND CHECK OF CONTRACTOR'S EMPLOYEES. Prior <br />to performing work in the City pursuant to the Agreement, all managers and employees of <br />Contractor, any independent contractors, and any subcontractors (collectively "Personnel") shall <br />be required to undergo a criminal background check consisting of a Florida Department of Law <br />Enforcement ("FDLE") and Florida Crime Information Center/National Crime Information Center <br />("FCIC/NCIC") criminal records check. Any Personnel not meeting this requirement will not be <br />permitted to perform work under this Agreement. The aforementioned criminal background check <br />shall be performed through the City's Human Resources Department, at the Contractor's sole cost <br />and expense. Failure of Contractor to comply with the provisions of this section, herein, shall be <br />cause for termination of the Agreement. <br />The Contractor's Personnel shall wear an identifiable uniform shirt provided by the Contractor, <br />and maintain a clean, neat appearance. <br />8. INSURANCE. Contractor shall, at its sole cost and expense, during the period of any <br />work being performed under this Agreement, procure and maintain the following minimum <br />insurance coverages to protect the City and Contractor against all loss, claims, damage and <br />liabilities caused by Contractor, its agents or employees, as more particularly set forth below: <br />(a) Commercial Liability Insurance. A Commercial Liability Insurance Policy shall be <br />provided which shall contain minimum limits of one million Dollars ($1,000,000.00) per <br />occurrence combined single limit for bodily injury liability and property damage liability <br />and shall contain minimum limits of two million Dollars ($2,000,000.00) per aggregate. <br />Coverage must be afforded on a form no more restrictive than the latest edition of the <br />Commercial Liability Policy, without restrictive endorsements, as filed by the Insurance <br />Services Office and must include: <br />i. Premises and/or operations. <br />ii. Independent Contractors. <br />iii. Products and/or Completed Operations for contracts. <br />iv. Broad Form Property Damage <br />V. Broad Form Contractual Coverage applicable to this specific Contract, <br />including any hold harmless and/or indemnification Contract. <br />Vi. Personal Injury Coverage with Employee and Contractual Exclusions <br />removed, with minimum limits of coverage equal to those required for <br />Bodily Injury Liability and Property Damage Liability. <br />(b) Business Automobile Liability. Business Automobile Liability shall be provided with <br />minimum limits of One Million Dollars ($1,000,000.00) per occurrence, combined single <br />limit for Bodily Injury Liability and Property Damage Liability. Coverage must be afforded <br />on a form no more restrictive than the latest edition of the Business Automobile Liability <br />Bettoli Trading Corporation d/b/a Bettoli Vending <br />583 <br />
