COMPANY PROFILE DECK - OCTOBER 2022QUALIFICATIONSPG. 06COMPANY PROFILE DECK - OCTOBER 2022• Our proof of insurance is attached in Appendix A <br />• Our W9 is attached in Appendix B <br />Prime 29, Inc. has met all minimum qualifications as detailed below: <br />Prime 29 Inc. is licensed to conduct business in the State of Michigan and will provide a certificate of authority <br />confirming status as a registered Florida business entity once processed by the Florida Department of State. <br />Applications with the Division of Alcoholic Beverages and Tobacco, Beverage License Department and Division of <br />Hotels and Restaurants are drafted and will be submitted upon acceptance of the award. All other licenses required <br />for the restaurant’s operations and key professional staff will also be applied for upon acceptance of the award. The <br />principals of the Firm currently operate the local restaurant known as Yakko San at 3881 NE 163rd St, North Miami <br />Beach, FL 33160 and therefore are familiar with the licensing requirements within the jurisdiction. <br />To further show the exemplary strength of the principals of Prime 29, by way of our <br />successful restaurant group (Prime Concepts), we have attached in Appendix C multiple <br />letters of reference from our landlords. <br />Lastly, the Firm has met all minimum requirements by submitting the attached financial <br />statements for the past three years as instructed in the Request for Proposal. <br />These documents are submitted in a separate closed file titled “Exempt from public records per §119.071, FLA.
