the coverage afforded shall not be canceled, materially changed or renewal refused until at least thirty <br />(30) calendar days written notice has been given to CITY by certified mail. <br />10.4.3 The CONTRACTOR shall procure and maintain at its own expense and keep in effect <br />during the full term of the Contract a policy or policies of insurance which must include the following <br />coverage and minimum limits of liability: <br />10.4.3.1 Professional Liability (Errors and Omissions) Insurance for <br />architectural and engineering services and the services of any other professional used in the <br />performance of the work of this Agreement in the amount of $1,000,000.00 with a deductible (if <br />applicable) not to exceed $5,000.00 per claim. The certificate of insurance for professional liability <br />shall reference any applicable deductible and the work of this Agreement. <br />10.4.3.2 Worker's Compensation Insurance for statutory obligations imposed <br />by Worker's Compensation or Occupational Disease Laws, including, where applicable, the United <br />States Longshoremen's and Harbor Worker's Act, the Federal Employers' Liability Act and the <br />Homes Act. Employer's Liability Insurance shall be provided with a minimum of One Hundred <br />Thousand and xx/100 dollars ($100,000.00) per accident. CONTRACTOR agrees to be responsible <br />for the employment, conduct and control of its employees and for any injury sustained by such <br />employees in the course of their employment. <br />10.4.3.3 Comprehensive Automobile Liability Insurance for all owned, non - <br />owned and hired automobiles and other vehicles used by the CONTRACTOR in the performance of <br />the work with the following minimum limits of liability: <br />$1,000,000.00 Combined Single Limit, Bodily Injury and Property <br />Damage Liability, per occurrence <br />10.4.3.4 Comprehensive General Liability Insurance (occurrence form) with <br />the following minimum limits of liability: <br />$1,000,000.00 Combined Single Limit, Bodily Injury and Property <br />Damage Liability, per occurrence <br />Coverage must be offered in a form no more restrictive than the latest edition <br />of the Comprehensive General Liability Policy without restrictive endorsements, as filed by the <br />Insurance Services Office and shall specifically include the following with minimum limits not less <br />than those required for Bodily Injury Liability and Property Damage Liability: <br />10.4.3.4.1 Premises and Operations; <br />10.4.3.4.2 Independent Contractors; <br />10.4.3.4.3 Product and Completed Operations Liability; <br />10.4.3.4.4 Broad Form Property Damage; <br />21 <br />
