The <br />project consisted of reconstructing approximately <br />4.1 miles of an existing 2 -lane rural road to a 4 -lane <br />divided urban roadway with four 12 -foot travel lanes <br />and two 4 -foot bicycle lanes. <br />• Roadway design to convert the existing 2 -lane rural roadway to a 4 -lane urban roadway <br />• Complete hydraulics analysis for the primary and secondary systems <br />• 0.6 mile realignment of the northern end of Belle Terre Parkway <br />• 0.12 mile two -lane curb and gutter extension of Buffalo Bill Drive from the existing Belle Terre Parkway <br />to the realigned Belle Terre Parkway <br />• 5 Signalized Intersections (Mast Arm) (interconnected) <br />• Four - Phased Maintenance of Traffic Plans <br />• Pond Siting Analysis <br />• Right -of -Way Acquisition services <br />• Traffic Volume Projections <br />• Signal Warrant Analyses <br />• Access Management Analysis <br />• Utility Relocation Coordination, including undergrounding 2 miles of FPL, Bright House, and ATT facilities <br />and 2.1 miles aboveground relocations <br />• Wetland and Floodplain Impacts - Permitting and Mitigation <br />Additional services include: <br />• Re- design of the City's water main and force main along the corridor (22,000 -LF of 12 " -20" water mains <br />and 20,000 -LF of 16" force main) <br />• Landscape and Irrigation design <br />• CPH developed a detailed public involvement program that included routine council presentations, <br />general public involvement, meetings with special interest groups <br />• Preparation of the Project Bid Documents, including Supplemental Specs and Technical Special <br />Provisions and Post Design Services <br />Client Contact: City of Palm Coast, Mr. Richard Adams, Public Works Director, 2 Utility Dr., Palm Coast, FL 32137, <br />Phone: 386.986.2351, Fax: 386.986.2391, E -Mail: radams @ci.palm- coast.fl.us <br />18 CPH, Inc. /RFQ No. 14 -07 -02 <br />
