CRAIG A. SMITH & ASSOCIATES PROPOSED SCOPE OF SERVICES P4009 GENERAL Craig A. Smith & Associates (CAS) proposes to accomplish the services as follows: ITEM N0. -- PAGE 2 -- S86 ROUTE SURVEY (BASIN A — 51 ACRES, MORE OR LESS) Survey will physically locate all above ground, visible improvements within the existing rights of way of the following streets: Bay Ave. from 174th St to 183rd St.; 178TH Dr. from Bay Ave. to its intersection with 178th St.; Church St.; 177th Dr.; 1781h St. and 179th Dr. from Bay Ave. to Atlantic Blvd.; 180th Dr., 1811t Dr.; 182nd Dr. and 1831 St. from Bay Ave. to Atlantic Blvd. Survey baseline will be established at 100 -foot intervals and cross sections taken to ten (10) feet beyond each right of way at fifty -foot intervals. Plan view elevations will be shown as relative to North American Vertical Datum of 1988 at each section and at pertinent points for facilitation of Engineering Design. All visible, above ground improvements and utilities will be located and shown. Trees 3" or greater will be shown with diameter at breast height (DBH) and common name if known by the Surveyor's agent. A base map will be created in current software parameters and provided to the Engineering Department as well as a Certified Map of Specific Purpose Survey adhering to the Standards of Practice for Surveying, (Chapter 5J —17) of the Florida Administrative Code, for submittal to entities of interest. $45,670.00 S87 ROUTE SURVEY - UTILITY SURFACE MARKS (BASIN A) Survey will physically locate all utilities as marked on the surface by the Craig A. Smith & Associates Locating Department within the existing rights of way of the following streets: Bay Ave. from 17411 St to 1831 St.; 178111 Dr. from Bay Ave. to its intersection with 178th St.; Church St.; 17711 Dr.; 17811 St. and 1791h Dr. from Bay Ave. to Atlantic Blvd.; 1801 Dr., 181St Dr.; 182"1 Dr. and 1831 St. from Bay Ave. to Atlantic Blvd. The information will be added to the Survey prepared under Task S86 above. $13,050.00 S88 ROUTE SURVEY — UTILITY TEST HOLES (SOFT DIGS) (BASIN A) Survey will physically locate the positions of up to 50 utility test holes (soft digs) within the existing rights of way of the following streets: Bay Ave. from 174th St to 183rd St.; 178TH Dr. from Bay Ave. to its intersection with 1781 St.; Church St.; 1771 Dr.; 1781 St. and 179th Dr. from Bay Ave. to Atlantic Blvd.; 1801h Dr., 181s1 Dr.; 182nd Dr. and 18311 St. from Bay Ave. to Atlantic Blvd. The information will be shown on the survey prepared under Task S86 above with utility data, i.e. type, size, material, inv., etc., for each based on reports provided to the Surveyor. $4,700.00 S90 PUMP STATIONS AND OUFALLS (BASIN A) Survey will physically locate all above ground, visible improvements within the areas where the two storm water pump stations may be placed; more particularly the corner of 181" drive and N.
