These certificates shall <br />contain a provision that the coverage afforded under the policies will not be canceled or materially <br />changed until at least thirty (30) days prior written notice has been given to the City by certified <br />mail. The City shall be named as an additional insured on the above-referenced policies. <br /> <br /> Provide: Additional Insured coverage on a Primary and Non-Contributory basis; Waiver of <br />Subrogation in favor of the City of Sunny Isles Beach. <br /> <br /> 2.10.3 Worker’s Compensation Insurance <br />Worker's Compensation Insurance for statutory obligations imposed by Worker's Compensation or <br />Occupational Disease Laws, including, where applicable, the United States Longshoremen's and <br />Harbor Worker's Act, the Federal Employers’ Liability Act and the Homes Act. Employer's Liability <br />Insurance shall be provided with a minimum of limits not less than: (1) $500,000 each accident for <br />bodily injury; (2) $500,000 each employee for bodily injury caused by disease; and (3) $500,000 bodily <br />injury caused by disease. Such policies shall cover all employees engaged in any Work under the <br />agreement. Bidder agrees to be responsible for the employment, conduct and control of its employees <br />and for any injury sustained by such employees in the course of their employment. <br />Provide: Waiver of Subrogation in favor of the City of Sunny Isles Beach and city of North Miami Beach. <br /> <br />2.10.4 Environmental Pollution Insurance <br />The Contractor shall carry an Environmental Pollution Insurance for pollution-related incidents, <br />including the cost of cleaning up a site after a pollution incident, with limits not less than $1,000,000 <br />per occurrence with deductible not greater than $100,000.00. An additional Form or endorsement to <br />the Commercial General Liability Insurance to include an Environmental Pollution Insurance coverage <br />providing the specified coverage, is acceptable. <br /> 2.10.5 Cancellation and Re-Insurance <br />If any insurance should be cancelled or changed by the insurance company or should any insurance <br />expire during the period of this contract, the Consultant shall be responsible for securing other <br />acceptable insurance to provide the coverage specified in this section to maintain coverage during <br />the life of this Contract. All deductibles must be declared by the Bidder and must be approved by <br />the City. At the option of the City, either the Bidder shall eliminate or reduce such deductible or <br />the Bidder shall procure a Bond, in a form satisfactory to the City, covering the same. <br />2.10.6 Builder’s Risk insurance <br /> <br />Builder’s Risk insurance for all construction performed on the site to the full value of the contract. <br />City of North Miami Beach and City of Sunny Isles Beach must be shown as an additional insured <br />with respect to this coverage. The mailing address of City of North Miami Beach 17011 NE 19th <br />Avenue, Suite 315, North Miami Beach, Florida 33162, as the certificate holder, must appear on <br />the certificate of insurance. <br /> <br />2.10.7 Equipment Floater insurance <br />Re-bid City of Sunny Isles Beach & NMB Water| Drainage Improvements & Watermain Replacement 16 <br /> <br />
