Contractor must provide Project Manager(s) with a hard copy and/or electronic copies of all forms <br />and documents prepared as part of the Quality Control Plan monitoring. <br />3.3 THE FOLLOWING SCOPE OF WORK APPLIES TO GROUP 2 — AS NEEDED <br />For services under Group 2, a written quote by the Contractor and a Purchase Order signed by the <br />City is required for Contractor to render services for the task order. <br />Under this agreement, the Contractor will perform repairs and as -needed maintenance as <br />specified by Project Manager, in accordance with conditions specified and the hourly contracted <br />rates. The Contractor shall assign a competent service technician, as per organizational chart, <br />specifically qualified to perform the particular task to each call. All provisions for diagnostic calls, <br />emergency calls, tools, and parts required to inspect and repair defective equipment shall be <br />included in the contract. <br />Scope for Group 2, may include but is not limited to: <br />• Replacement of any damages pull boxes or lids <br />• Repair or repair any underground light pole concrete base <br />• Repair of repair any service point damage by accident or due to age of material <br />• Retrofit or upgrade any system in the street lights <br />• Repair or replace step down transformers or GFCI in the poles or removal of any <br />decoration accessories for Holiday season. <br />• Install any missing wiring <br />• Perform any repairs related to natural disasters. <br />UNDERGROUND DAMAGES <br />Contractor must make repairs if damages are caused by third parties such as FPL, AT&T, Cable <br />TV, other utilities or any private contractor. Contactor must notify City Project Manager <br />and take pictures and notify Project Manager of identified damages prior to repairing. An <br />estimated labor cost per hour, equipment, and materials is required for approval by Public <br />Works -Operations, and an invoice must be provided to the City at the end of the month. <br />If City employees cause damages, the City will be responsible for the damages. <br />
