<br />SFM Services, Inc.
<br />The registered agent and authorized persons for SFM Janitorial Services, LLC., are as follows:

<br />SFM Services, Inc., is the parent company of SFM Janitorial Services, LLC., and the ownership interests

<br />LITIGATION
<br />SFM Janitorial Services, LLC., and its related entities may be included from time to time in litigation
<br />brought against its clients due to the nature of its business; there is no prior or pending litigation that
<br />would have a material adverse effect on SFM’s ability to deliver services under any award of the RFP.
<br />There is no pending or prior litigation in which the Team has been averse to any of its clients or
<br />governmental entities.
<br />SFM Janitorial Services, LLC., does not have anycivil, criminal, administrative, bankruptcy or other similar
<br />proceeding was filed against our company.
<br />No city, county, or other governmental entity has terminated a contract with SFMconcerning a similar
<br />scope of work/service.
<br />SFM Janitorial Services, LLC., has not hadanyadministrative fines, liquidated damages, civil penalties,
<br />other penalties (collectively “penalties”) assessed against or deducted from ourpayments under a
<br />contract with a city, county, or governmental entity for a similar scope of work/service to this RFP.
<br />SFM Janitorial Services, LLC., has not hadto paymore than ten thousand dollars ($10,000) to settle a
<br />dispute with an organization and/or a governmental entity concerning ourperformance under contract
<br />for similar scope of work/service.
<br />SFM Janitorial Services, LLC., nor any of its owners, officers, subsidiaries, or affiliates have been excluded,
<br />disqualified, or disbarred by any federal, state or local government or agency since January 1, 2013.
<br />SFM Janitorial Services, LLC., nor any of its owners, officers, subsidiaries, or affiliates have in the last ten
<br />(10) years refused to enter into a contract for a similar scope of work to this RFP after an award had been
<br />made to the Proposer; or failed to complete a contract for a similar scope of work/service to this RFP; or
<br />been declared to be in default in any contract for a similar scope of work to this RFP.
