7 Notice will be given to include name, address, phone number and a map to each Superintendent and Foreman for nearest Urgent medical centers and/or hospitals in counties other than those listed here. ACCIDENT PROCEDURES RIC-MAN will not accept or acknowledge the occurrence of any accident or injury which is not reported immediately when it occurs. Upon receiving a report of accident or injury, the supervisor must send the involved employee(s) for first aid attention; transportation will be provided or arranged for any serious injury. The supervisor must investigate and prepare a written accident report describing the event and involved employee(s) and procedures followed. The supervisor's investigation shall document the cause of the accident or injury and include an effort to ensure corrective and/or preventative action is taken immediately where appropriate to prevent further injury and avoid recurrence. The supervisor’s report must be submitted to the main office on the day the accident or injury is reported. The supervisor’ investigation will include an interview of all employee(s) who were involved in or witnessed the incident and a determination of what work or activity was being performed, the manner in which it was being performed, and the nature and extent of any injury. The investigation should include evaluation of the safety procedures followed and the presence of appropriate or required safety equipment A written witness statement should be obtained regarding the events from all eyewitnesses.
