7 <br /> <br />Notice will be given to include name, address, phone number and a map to each <br />Superintendent and Foreman for nearest Urgent medical centers and/or hospitals in counties <br />other than those listed here. <br /> <br /> ACCIDENT PROCEDURES <br /> <br />RIC-MAN will not accept or acknowledge the occurrence of any accident or injury which is not <br />reported immediately when it occurs. Upon receiving a report of accident or injury, the <br />supervisor must send the involved employee(s) for first aid attention; transportation will be <br />provided or arranged for any serious injury. <br /> <br />The supervisor must investigate and prepare a written accident report describing the event <br />and involved employee(s) and procedures followed. The supervisor's investigation shall <br />document the cause of the accident or injury and include an effort to ensure corrective and/or <br />preventative action is taken immediately where appropriate to prevent further injury and avoid <br />recurrence. The supervisor’s report must be submitted to the main office on the day the <br />accident or injury is reported. <br /> <br />The supervisor’ investigation will include an interview of all employee(s) who were involved <br />in or witnessed the incident and a determination of what work or activity was being <br />performed, the manner in which it was being performed, and the nature and extent of any <br />injury. The investigation should include evaluation of the safety procedures followed and <br />the presence of appropriate or required safety equipment A written witness statement <br />should be obtained regarding the events from all eyewitnesses. <br /> <br />UPDATE, JUNE 2019 <br /> <br />This company has safety rules to keep you from getting hurt on the job.
