P ROPOSAL F ORM 18: E QUALIS G ROUP A DMINISTRATION A GREEMENT D ECLARATION <br />Attachment A - Sample Administration Agreement of this solicitation is for reference only. Contracting <br />with Equalis Group and the Winning Supplier will occur after contract award. <br />Execution of the Administration Agreement is required for the Master Agreement to be administered by <br />Equalis Group. Attachment A - Sample Administration Agreement defines i) the roles and responsibilities of <br />both parties relating to marketing and selling the Program to current and prospective Members, and ii) the <br />financial terms between Equalis Group and Winning Supplier. <br /> <br />Redlined copies of this agreement should not be submitted with the response. Should a respondent be <br />recommended for award, this agreement will be negotiated and executed between Equalis Group and the <br />respondent. Respondents must select one of the following options for submitting their response. <br /> Bidder agrees to all terms and conditions outlined in the Attachment A - Sample Administration <br />Agreement. <br /> Bidder wishes to negotiate directly with Equalis Group on terms and conditions outlined in the Sample <br />Administration Agreement.
