December 7, 2022
Mr. Ian DeMello
Shubin & Bass, PA
46 S.W 1St Street, Third Floor
Miami, FL 33130
Re: Report for Concrete Core Physical and Chemical Analyses
334 Atlantic Avenue
334 Atlantic Ave
Sunny Isles Beach, Florida 33160
NV5 Project No.: 18005.00
Dear Mr. DeMello:
NV5, Inc. submits this report in fulfillment of the scope of services described in our proposal 22-0975
dated October 25, 2022. This report contains the data collected and describes the procedures used.
PROJECT INFORMATION
The project site is located at 334 Atlantic Ave, Sunny Isles Beach, Florida. The site is bounded by a
single-family home to the east, by Atlantic Isle Road to the north, and by Biscayne Bay to the south and
west. The site is approximately trapezoidal in shape.
PURPOSE
The purpose of our services on this project is to perform laboratory compressive strength and chemical
analyses on the concrete core and present a report summarizing the results.
SAMPLE COLLECTION
NV5 was instructed by the client to retrieve cores from three different structural elements. A Ground
Penetrating Radar (GPR) was performed to determine the adequate locations to extract the cores. Five
(5) cores were extracted by NV5 personnel and were bagged within the time limit indicated I ASTM
C42-20 and transported to NV5 laboratory for testing. The core locations are summarized in Table 1.
The concrete cores dimensions were 2.77 -inches in diameter and varied in length from 5.92- to 6.98 -
inches. We note that the concrete cores diameter is less than that recommended in ASTM C42
Standard Test Method for Obtaining and Testing Drilled Cores and Sawed Beams of Concrete.
14486 Commerce Way I Miami Lakes, FL 33016 1 www.NV5.com I Office 305.666.3563 1 Fax 305.666.3069
CONSTRUCTION QUALITY ASSURANCE - INFRASTRUCTURE - ENERGY - PROGRAM MANAGEMENT - ENVIRONMENTAL
Page 33 of 98 44
