THE CITY OF SUNNY ISLES BEACH <br />PROJECT NO. 21-1011 <br />BUILDING DEPARTMENT RELOCATION TO ANNEX <br /> <br />Project No. 21-1011 November 2024 028200 Asbestos Abatement-18 <br /> <br />Describe, in a narrative format, specific work procedures, exposure/contamination controls, and <br />engineering controls. This description should include, but not be limited to, the following types <br />of information: <br /> <br />OSHA Class I, II, III, IV <br />Negative pressure enclosure <br />Wet methods <br />Glove bagging <br />Respiratory Protection <br />HEPA vacuums <br />Containment (polyethylene barriers) without negative pressure <br />Solvent removal of mastic <br />Other procedures (list) <br /> <br />VII. Respiratory Protection and Protective Clothing/Personal Protective Equipment: <br /> <br />List all respiratory protection, including types and manufacturers, which are anticipated for this <br />project. Identify the phases of the project for which respirators will be required or likely to be <br />required. <br /> <br />List all personal protective equipment anticipated to be used on the project. <br /> <br /> VIII. Decontamination/Hygiene Facilities: <br /> <br /> Identify the types and locations of decontamination or hygiene facilities to be used on this <br />project. Specify use of disposable towels, soap, hot and cold water, and other supplies, all to be <br />provided by the Subcontractor. <br /> <br /> Specify the required use of the facilities, including use of the facilities prior to eating, drinking, <br />and smoking, and before leaving the project site. <br /> <br /> Describe handling or treatment of asbestos-contaminated solid waste and wastewater. <br /> <br />IX. Air Monitoring Data: <br /> <br />Identify general worker air monitoring protocol to be followed on this project, including worker <br />category classifications, frequency of monitoring, anticipated laboratory to be used for sample <br />analysis, and how pumps are calibrated and worn by employees. Identify competent person who <br />will oversee Subcontractor's air monitoring. <br /> <br />X. Worker Training and Qualifications: <br /> <br />Provide the Subcontractor's worker documents as an attachment to the Asbestos Work Plan. <br />Include each worker’s current AHERA training records, respiratory protection training and fit- <br />testing certification, and asbestos and respiratory protection medical clearances. <br /> <br />XI. Notification: <br /> <br /> Describe all arrangements made on multi-employer work sites to inform affected employers <br />about the asbestos project. Attach copies of any notifications. <br />
