Backing Rod <br />9. Bond Breaker <br />10. Waterstop <br />11. Slip Dowels <br />12. PVC Tubing <br />1.5 RESPONSIBILITIES: <br />A. The CONTRACTOR is fully responsible for the design and construction of all forms and <br />falsework to be in compliance with all applicable OSHA requirements, and the requirements <br />of all agencies having jurisdiction on the project. The submission of design details and <br />calculations for falsework is for information only. <br />B. The CONTRACTOR shall prepare adhesion and cohesion test specimens for construction <br />joint sealant as required herein, at intervals of 5 working days while sealants are being <br />installed. <br />C. The sealant material shall show no signs of adhesive or cohesive failure when tested in <br />accordance with the following procedure in laboratory and field tests: <br />1. Sealant specimen shall be prepared between 2 concrete blocks (1-inch by 2-inch <br />by 3-inch). Spacing between the blocks shall be 1-inch. Coated spacers (2-inch <br />by 1-1/2-inch by 1/2-inch) shall be used to insure sealant cross-sections of 1/2- <br />inch by 2 inches with a width of 1-inch. <br />2. Sealant shall be cast and cured according to manufacturer's recommendations <br />except that curing period shall be not less than 24 hours. <br />3. Following curing period, the gap between blocks shall be widened to 1-1/2-inch. <br />Spacers shall be used to maintain this gap for 24 hours prior to inspection for <br />failure. <br />1.6 CERTIFICATIONS: <br />A. Form materials, which may remain or leave residues on or in the concrete, shall be certified <br />as compliant with NSF 61. <br />B. Joint materials shall be certified as compliant with NSF 61. <br />C. CONTRACTOR shall submit certified test reports from the sealant manufacturer on the <br />actual batch of material being supplied indicating compliance with requirements herein <br />before the sealant is used on the job. <br />1.7 INSPECTIONS: <br />A. Falsework shall be inspected for conformance with the accepted submittal. No workers <br />will be allowed to use falsework for access and no concrete placement to related forms will <br />be permitted until the falsework is inspected by the CONTRACTOR for conformance with <br />the submittals and appropriately tagged. No variations or alterations to falsework, as <br />compared to the reference submittal, will be allowed without certification of the variation <br />by the original Professional Engineer.
