All required signatures shall be manual <br />by an authorized representative who has the legal authority to bind the CONTRACTOR in <br />contractual obligations. Each page of the bid should state the name of the CONTRACTOR, the bid <br />number, and the page number. The City reserves the right to request additional data or material <br />to support bid. All material submitted in response to the ITB will become the property of the City. <br /> <br />LABEL EACH SECTION AS NUMBERED <br /> <br />Minimum Qualification Requirements (MQRS) <br /> <br />Bidders must read the MQRs first to ensure you meet these requirements in order to provide <br /> a response to this ITB. Bidders that do not meet all the MQRs stated will be determined non- <br /> responsive and disqualified from the evaluation process and will not be considered. Bidder <br /> must provide all MQRs as requested, in addition to the information requested below. <br /> <br />The proposal must be in the following format specifically addressing each criterion below: <br /> <br />1. Company Information <br />In response to this Proposal, all Contractors must provide the following: <br /> Name of Agency/Company (including any "Doing Business As" names) <br /> Company Locations <br /> Internet Web Site Address (if any) <br /> Office address and telephone number, email address <br /> Proof of insurance <br /> Bidder must submit a copy of Florida Division of Corporations Sunbiz report with <br />your company registered as active. <br /> W9 <br /> Bonding Capacity Letter <br /> Two years of its most recent certified business financial statements as of a date <br />not earlier than the end of the Proposer’s preceding official tax accounting <br />period, together with a statement in writing, signed by a duly authorized <br />representative, stating that the present financial condition is materially the <br />same as that shown on the balance sheet and income statement submitted, or <br />with an explanation for material change in the financial condition. <br /> <br />2. Qualifications <br />Proposer’s relevant experience, qualifications and past performance <br /> <br /> An explanation of why the Contractor is the best qualified to perform the contract <br />and demonstrate its qualifications including an item-by-item disclosure outlining <br />how the Contractor meets or exceeds the requirements of this ITB.
