required by this Contract shall be issued by an insurance company, acceptable to the City and <br />authorized to do business in the State of Florida with an A.M:Best rating of B+ or better. <br />13.2. Commercial General Liabilitv. Contractor agrees to maintain Commercial General <br />Liability at a limit of liability not less than $1,000,000.00 Each Occurrence $2,000,000.00 Annual <br />Aggregate on a primary and non-contributory basis, including the City of Sunny Isles Beach as an <br />Additional Insured. Coverage must be offered in a form no more restrictive than the latest edition <br />of the Comprehensive General Liability policy, without restrictive endorsements, as filed by the <br />Insurance Services Office, and shall specifically include the following with minimum limits not less <br />than those required for Bodily Injury Liability and Property Damage Liability: <br />• Premises and Operation; <br />• Independent Contractors; <br />• Products and/or Completed Operations Hazard; <br />• Broad Form Property Damage <br />• Broad Form Contractual Coverage applicable to this Contract, including any hold <br />harmless and/or Indemnification Agreement; <br />• Personal Injury Coverage with Employee and Contractual Exclusions removed, with <br />minimum limits of coverage equal to those required for Bodily Injury Liability and <br />Property Damage Liability; <br />• $5,000,000 Umbrella/Excess Liability over and above and following the above <br />mentioned General Liability & Business Automobile Liabilities. <br />13.3. Business Automobile Liabilitv. Contractor agrees to maintain Business Automobile <br />Liability at a limit of liability not less than $1,000,000.00 per Occurrence combined single limit for <br />Bodily Injury Liability and Property Damage Liability. Coverage must be afforded on a form no more <br />restrictive than the latest edition of the Business Automobile Liability policy, without restrictive <br />endorsements, as filed by the Insurance Services Office, and must include: Owned, Non -Owned & <br />Hired automobiles, and Employer's Non -City ship. <br />13.4. Worker's Compensation & Employer's Liabilitv. Contractor agrees to maintain <br />Worker's Compensation & Employers Liability Insurance in accordance with Florida Statutes, <br />including Employer's Liability of $1,000.000.00 <br />13.5. Environmental Pollution Insurance. Contractor shall maintain Environmental <br />Pollution Insurance for pollution -related incidents, including the cost of cleaning up a site after a <br />pollution -related incident, with limits not less than $500,000.00 per occurrence, with a deductible <br />not greater than $100,000.00. An additional form or endorsement to the Commercial General <br />Liability Insurance to include Environmental Pollution Insurance coverage providing the specified <br />coverage is acceptable. <br />13.6. Builder's Risk Insurance. Contractor, prior to notice to proceed or commencement <br />of Work, whichever occurs first, agrees to maintain Builder's Risk insurance with an amended policy <br />period not less than the Contract Time, plus three (3) months, affording coverage to protect the <br />insurable interests of the City, Contractor, sub -contractors, including property acquired under a <br />Page 31 of 42 <br />381 <br />
