• Broad Form Contractual Coverage applicable to this Contract, including any hold <br />harmless and/or Indemnification Agreement; <br />• Personal Injury Coverage with Employee and Contractual Exclusions removed, with <br />minimum limits of coverage equal to those required for Bodily Injury Liability and <br />Property Damage Liability; <br />• $5,000,000 Umbrella/Excess Liability over and above and following the above <br />mentioned General Liability & Business Automobile Liabilities. <br />13.3. Business Automobile Liability. Contractor agrees to maintain Business Automobile <br />Liability at a limit of liability not less than $1,000,000.00 per Occurrence combined single limit for <br />Bodily Injury Liability and Property Damage Liability. Coverage must be afforded on a form no more <br />restrictive than the latest edition of the Business Automobile Liability policy, without restrictive <br />endorsements, as filed by the Insurance Services Office, and must include: Owned, Non -Owned & <br />Hired automobiles, and Employer's Non -City ship. <br />13.4. Worker's Compensation & Employer's Liability. Contractor agrees to maintain <br />Worker's Compensation & Employers Liability Insurance in accordance with Florida Statutes, <br />including Employer's Liability of $1,000.000.00 <br />13.5. Environmental Pollution Insurance. Contractor shall maintain Environmental <br />Pollution Insurance for pollution -related incidents, including the cost of cleaning up a site after a <br />pollution -related incident, with limits not less than $500,000.00 per occurrence, with a deductible <br />not greater than $100,000.00. An additional form or endorsement to the Commercial General <br />Liability Insurance to include Environmental Pollution Insurance coverage providing the specified <br />coverage is acceptable. <br />13.6. Builder's Risk Insurance. Contractor, prior to notice to proceed or commencement <br />of Work, whichever occurs first, agrees to maintain Builder's Risk insurance with an amended policy <br />period not less than the Contract Time, plus three (3) months, affording coverage to protect the <br />insurable interests of the City, Contractor, sub -contractors, including property acquired under a <br />sales tax incentive program, property in transit, and property on or off -premises, which shall <br />become part of the Work. Coverage shall be written on an All -Risk, Replacement Cost, and <br />Completed Value Form basis in an amount at least equal to one -hundred 100% of the projected <br />completed value of the Work, as well as subsequent modifications of that sum due to Change <br />Orders, with a deductible of not more than $50,000.00 per claim. . <br />13.7. Additional Insured Endorsement. Contractor agrees to endorse the City as an <br />Additional Insured on the Commercial General Liability with the following Additional Insured <br />endorsement, or similar endorsement providing equal or broader Additional Insured coverage, the <br />CG 2010 07 04 or GC 2010 0413 Additional Insured - Owners, Lessees, or Contractors — Scheduled <br />Person or Organization endorsement in combination with the additional endorsement GC 20 3710 <br />01 or GC 20 37 04 13 Additional Insured — Owners, Lessees, or Contractors — Completed <br />Operations. The name of the organization endorsed as Additional Insured for all endorsements shall <br />read "City of Sunny Isles Beach". <br />Page 32 of 44 <br />
