FLOAO''•' AGREEMENT BETWEEN THE CITY OF <br />Cr/r or fuN'NV <br />SUNNY ISLES BEACH AND EAGLE EYE OFFICIATING, LLC <br />CONTRACT NO.2023-6593 <br />THIS AGREEMENT (hereinafter referred to as the "Agreement") is made in duplicate, <br />this 2Aday of j�j`_, 2023, by and between the CITY OF SUNNY ISLES <br />BEACH, Florida, (hereinafter referred to as "City"), and EAGLE EYE OFFICIATING, LLC, <br />a corporation authorized to do business in the State of Florida (hereinafter referred to as <br />"Contractor") whose Federal I.D. # is 47-4619258. <br />RECITALS <br />WHEREAS, the City is in need of a contractor to provide Sports Officiating and <br />Scorekeeping Services for Youth Athletic Leagues for soccer, volleyball, and basketball games <br />held by the City's Cultural and Community Services Department ("Services"); and <br />WHEREAS, the City issued an Invitation to Bid ("ITB") for Sports Officiating and <br />Scorekeeping Services for Youth Athletic Leagues, and Contractor submitted a response thereto, <br />a copy of which is attached hereto and incorporated herein as Attachment "A"; and <br />WHEREAS, Contractor was selected as the lowest responsible, responsive bidder in <br />response to the ITB; and <br />WHEREAS, Contractor is willing and able to perform the services based on the terms and <br />conditions set forth in ITB for Sports Officiating Services for Youth Athletic Leagues, a copy of <br />which is attached hereto and incorporated herein as Attachment "B"; and <br />WHEREAS, the City desires to enter into this Agreement with Contractor to provide the <br />Services, and prices shall remain fixed and firm throughout the term of the Agreement. <br />NOW THEREFORE, in consideration of the promises and the mutual covenants herein, <br />the parties agree as follows: <br />1. RECITALS. The Recitals set forth above are hereby incorporated into this agreement and <br />made a part hereof for reference. <br />2. THE CONTRACT DOCUMENTS. The following documents (collectively "Contract <br />Documents") are hereby incorporated into and made part of this Agreement: <br />A. Invitation to Bid No. 22-12-02, "Sports Officiating and Scorekeeping Services for <br />Youth Athletic Leagues", including any and all addenda, prepared by the City <br />("RFP" or Attachment "A") <br />B. The Contractor's response to the RFP (Attachment `B") <br />C. Contractor's Scrutinized Vendors Affidavit (Attachment "C") <br />D.
