Active Sunbiz Registry As of the submission deadline, CGA is registered with the Florida Secretary of State through Sunbiz and is in “Active” standing (Document # M17373). Later in this tab, you will find an image of this registry in the Occupational License Registration section. II. Florida Architecture License CGA is not bidding on the Architects category. III. Florida Engineering License CGA holds an Engineering Registry issued by the State of Florida Department of Business and Professional Regulations (DBPR). Our registry number is 514; you will find an image of this registry in the Occupational License Registration section later in this tab. Our proposed team members with PE licenses include Jenna Martinetti, Nicholas “Nico” Kanelidis Heidi Belle Henderson, James “Jimmy” Messick, Pablo Chon Kan-Munoz, Salman Rathore, Erik Padron, Hugo DaSilva, and Jorge Cervantes In addition, four (4) of our Sub-Consultants have proposed team members with PE licenses. On the following pages, we present images of the PE licenses of CGA’s proposed team members. You will find images of the PE licenses of our Sub-Consultants’ proposed team members in the Appendix at the end of this document. IV. Leadership in Energy and Environmental Design (LEED) Proposed team member Tammy Cook-Weedon holds numerous licenses and certifications, including a current and valid LEED AP+C. Additionally, both of the proposed team members from Delta G Consulting Engineers, Inc. (our Sub-Consultant for Mechanical, HVAC & Plumbing Engineers) hold LEED certifications. On the following pages, we present an image of Ms. Cook-Weedon’s LEED certification. You will find images of the LEED certifications of our Sub-Consultants’ proposed team members in the Appendix at the end of this document. V. Surveyors and Mapping CGA holds a Board of Professional Surveyors and Mappers Certificate of Authorization issued by the Florida Department of Agriculture and Consumer Services (FDACS). Additionally, proposed team member Steven M. Watts holds a Professional Surveyor and Mapper (PSM) license issued by the FDACS. On the following pages, we present an image of Mr. Watts’ PSM license. Minimum Qualification Requirement #2: Experience Within the past five (5) years, 2020-2025, CGA far surpasses the City’s requirement of three (3) government contracts of similar size and scope as to the requested services outlined in this RFQ. You will find more information about which discipline(s) each questionnaire represents in Tab 5. Combined, these 4 questionnaires in which the Proposer served as primary consultant for projects similar in size and scope within the last five (5) years, 2020 - 2025.
