The respondent must provide a copy of the State Certificate of good <br />standing listing the officers of the company. If the signatory is not one of the officers listed on the <br />State Certificate, the respondent must provide one of the following forms of evidence of Signatory <br />Authority with its response: <br /> <br />a. A copy of firm’s Articles of Incorporation listing the approved signatories of the corporation; <br />b. A copy of a resolution listing the members of staff as authorized signatories for the firm; <br />and <br />c. A letter from a corporate officer listing the members of staff that are authorized signatories <br />for the firm. <br /> <br />H. Forms and Attachments <br /> <br />a. Enclosed Forms; <br />b. Respondent must complete, sign as required, and submit the Addenda and all forms in the <br />following Section 6 as part of its submittal; <br />c. Firm’s Current Certificate(s) of Insurance; and <br />d.
