The Contractor shall not leave equipment on the right -of-way overnight except where authorized by <br />the City and all safety measures have been implemented. Note: The Contractor, with 24-hour prior <br />written permission from the City, may perform work during evening hours in the right-of-way or <br />easement, after a Maintenance of Traffic plan has been submitted and approved by the City. All cost <br />associated with M.O.T will be the responsibility of the Contractor. <br /> <br />9. The Contractor shall be solely responsible for disposal of all debris, and disposal shall be only in a City <br />approved lined landfill in accordance with all Federal, State and Local rules and regulations in effect <br />at the time of disposal. Cost involved with the disposal of the debris shall be included in the contract <br />unit price for cleaning. <br /> <br />10. All areas designated for repairs shall be videotaped at the completion of the requested service. A <br />detailed report (as-built) including video, drawing of actual site conditions (specifying repair points), <br />size of pipes, linear footage, manholes found, pictures and work performed must be submitted as a <br />complete package after completion. Videotaping will be paid for as individual bid items/line items in <br />the bid item schedule. The cost of required reports shall be included in the price for inspections. <br /> <br />11. EXECUTION - The contractor shall carry out his operation in strict accordance with all OSHA and <br />manufacturers safety requirements. In addition, the Contractor shall conform to the Florida <br />Department of Transportation’s (F.D.O.T.) most current editions for “Roadway and Traffic <br />Design Standards” for Design Construction, Maintained Systems and the FHWA “Manual of Uniform <br />Traffic Control Devices for Streets and Highways.” A proposed traffic control plan shall be <br />submitted to the City prior to starting work. <br /> <br />12. EQUIPMENT - Equipment shall conform to the FDOT “Standard Specifications,” latest edition except <br />where authorized by the City. <br /> <br />13. DISPOSAL OF DEBRIS <br />a. Under no circumstances shall solids be dumped onto the ground surface, streets or into ditches, <br />catch basins or storm drains. <br /> <br />b. All solids or semi-solids resulting from the operations shall be removed from the site by the <br />Contractor. Trucks hauling solids or semi-solids from the site shall be watertight so that no <br />leakage or spillage will occur. <br /> <br />c. Disposal shall be a suitable site selected by the Contractor and approved by the appropriate <br />jurisdictional personnel. <br /> <br />14. MATERIALS - Materials must meet the requirements of the FDOT “Standard Specifications,” latest <br />edition, except where authorized by the City. <br /> <br />
