Russo, City Manager <br /> FROM: Elka Linton-Dorsett, Capital Projects Manager <br /> DATE: 4/21/2016 <br /> A Resolution Approving the Fifth Amendment with Calvin, <br /> RE: Giordano &Associates, Inc. for the North Bay Road <br /> Bridge, North Seawall and Mitigation <br /> RECOMMENDATION: <br /> Staff is recommending the City Commission approve the attached <br /> resolution. <br /> REASONS: <br /> In July of 2010, the City entered into a contract with Calvin Giordano <br /> and Associates, <br /> Inc. for the design of the design and permitting of the North Bay Road <br /> Pedestrian Bridge. <br /> To date, the City Commission has approved four(4)amendments to the <br /> original contract, for a total contract value of$984,489.25 <br /> The proposed Work Authorization No. 175 will be the Fifth Amendment <br /> the existing agreement, allowing Calvin Giordano and Associates, Inc. <br /> and associated sub-consultants to provide to provide Design, Survey, <br /> and Materials Testing as well as additional Construction Management <br /> and Inspection Services for the Pedestrian and Emergency Vehicular <br /> Bridge. <br /> A total amount not to exceed Eight Hundred Thirty-Four Thousand One <br /> 127 <br />
