<br />VI. <br /> <br />INSURANCE <br /> <br />1. <br /> <br />Modification and Changes <br /> <br />The Provider shall notify the County of any intended changes in <br />insurance coverage, including but not limited to any renewals of <br />existing insurance policies. Upon review of the Provider's Scope of <br />Services (Attachment A) by the County's Risk Management Division, the <br />County may increase, decrease, waive or modify any of the following <br />insurance requirements. Any request by the Provider to decrease, waive <br />or modify any of the following insurance requirements must be approved <br />in writing by the County's Risk Management Division. <br /> <br />2. <br /> <br />Minimum Insurance Requirements: Certificates of Insurance <br /> <br />The Provider shall furnish to Miami-Dade County, c/o Contracts <br />Management Division, 111 N.W. First Street, Suite 2130, Miami, FL <br />33128-1912, Certificate(s) of Insurance indicating that insurance <br />coverage has been obtained which meets the requirements as outlined <br />below: <br /> <br />a. Worker's Compensation Insurance for all employees of the <br />Provider as required by Florida Statute 440. <br /> <br />b. Public Liability Insurance on a comprehensive basis in an <br />amount not less than $300,000 combined single limit per <br />occurrence for bodily injury and property damage. Dade <br />County must be shown as an additional insured with <br />respect to this coverage. <br /> <br />c. Automobile Liability Insurance covering all owned, <br />non-owned and hired vehicles used in connection with the <br />Services, in an amount not less that $300,000 combined <br />single limit per occurrence for bodily injury and <br />property damage. <br /> <br />3. <br /> <br />Classifications and Ratings <br /> <br />rhe insurance coverage required shall include those classifications, as <br />listed in standard liability insurance manuals, which most nearly <br />reflect the Services or operations described ln the Scope ot Services <br />\Attachment A). All insurance policies required above shall be issued <br />by companies authorized to do business under the laws of the State of <br />Florida, with the following qualifications: <br /> <br />The company must be rated no less that "B" as to management, <br />and no less than "Class V" as to financial strength by the <br />latest edition of Best's Insurance Guide, published by A.M. <br />Best Company, Oldwick, New Jersey, or its equivalent, subject <br />to the approval of the County Risk Management Division. <br /> <br />-3- <br />
