<br />B. Amount Of Supplemental Life Insurance <br />See the Coverage Features for the schedule of Supplemental Life Insurance. If we approve Evidence Of <br />Insurability, we will issue a notice showing the amount of Supplemental Life Insurance we have approved. <br /> <br />C. Increases In Supplemental Life Insurance <br />You must apply in writingfor any increase in Supplemental Life Insurance on your life or on your Spouse's life. <br /> <br />Subject to the Active Work Provisions, an increase in your Supplemental Life Insurance becomes effective <br />on the date we approve your Evidence Of Insurability. <br /> <br />An increase in your Spouse's Supplemental Life Insurance becomes effective on the date we approve your <br />Spouse's Evidence Of Insurability. <br /> <br />D. Decreases In Supplemental Life Insurance <br />A decrease in Supplemental Life Insurance because of a change in age becomes effective on the first day of the <br />calendar month coinciding with or next following the change in age. <br /> <br />Any other decrease in Supplemental Life Insurance becomes effective on the first day of the calendar month <br />coinciding with or next following the date the Policyowner or Employer receives your written request for the <br />decrease. <br /> <br />E. Suicide Exclusion: Supplemental Life Insurance <br />If death results from suicide or other intentionally self- inflicted Injury, while sane or insane, 1 and 2 below <br />will apply. <br /> <br />1. The amount payable will exclude the amount of Supplemental Life Insurance which has not been <br />continuously in effect for at least 2 years on the date of death. <br /> <br />2. . We will refund all premiums paid for Supplemental Life Insurance which is excluded from payment <br />under this suicide exclusion. <br /> <br />F. Becoming Insured For Supplemental Life Insurance <br />1. Eligibility <br /> <br />You and your Spouse are eligible if you are insured for Life Insurance. <br /> <br />2. Effective Date <br /> <br />You must apply in writing for Supplemental Life Insurance on your life or on your Spouse's life and agree <br />to pay premiums. <br /> <br />Subject to the Active Work Provisions, your Supplemental Life Insurance becomes effective on the <br />date we approve your Evidence Of Insurability. <br /> <br />Your Spouse's Supplemental Life Insurance becomes effective on the date we approve your Spouse's <br />Evidence Of Insurability. <br /> <br />G. When Supplemental Life Insurance Ends <br />Supplemental Life Insurance ends automatically on the earliest of: <br /> <br />1. The date all your Life Insurance ends; <br /> <br />2. The date the last period ends for which you made a premium contribution for the Supplemental Life <br />Insurance; <br /> <br />3. The date you die; and <br />4.
