A detailed statement of the corporate or other business entity <br />organization of the proposed transferee, including but not limited to <br />the following: <br /> <br />(a) The names, business addresses, state of <br />residence and country of citizenship of all general <br />partners and/or corporate officers and directors of <br />the proposed transferee. <br /> <br />(b) The names, business addresses, state of <br />residence and country of citizenship of all persons <br />and entities having, controlling, or being entitled to <br />have or control ten percent or more of the ownership <br />of the proposed transferee and the respective <br />ownership share of each such person or entity. <br /> <br />(c) The names and addresses of any subsidiary of <br />the proposed transferee and of any other business <br />entity owning or controlling in whole or in part or <br />owned or controlled in whole or in part by the <br />proposed transferee, <br /> <br />(d) A detailed and complete financial statement <br />of the proposed transferee, prepared by a certified <br />public accountant if audited statements were made, <br />and if not, by a duly authorized financial officer of <br />the proposed transferee, for the three fiscal years <br />immediately preceding the date of the request for <br />transfer approval and a letter or other acceptable <br />evidence in writing from a duly authorized officer of <br />the proposed transferee setting forth a clear and <br />accurate description of the amount and sources of <br />funding for the proposal transaction and its <br />sufficiency to provide whatever capital shall be <br />requires by the proposed transferee to construct, <br />install, rebuild, maintain and operate the proposed <br /> <br />-23- <br /> <br />"- <br /> <br />p~ <br />
