List the facilities which are addressed on page 5 of this application which are identified in the <br />priority ranked index clusters of outdoor facilities needs for renovation and/or new construction <br />identified within the applicant's population density as set forth in the Department's study entitled <br />"Infrastructure Assessment of Local Government Recreation and Park Department Facility Needs <br />in the State of Florida" effective December 1995.
