<br />APPENDIX 1 <br /> <br />CHECKLIST FOR SURFACE WATER <br />MANAGEMENT PERMIT APPUCATIONS <br /> <br />I. Site Information including: <br /> <br />A. Detailed location sketch. <br /> <br />B. Topographic map (with contours) of the site and adjacent hydrologically related <br />areas (minimum of 100 feet from project boundaries), which shall include location <br />and description of bench marks (minimum of one per major water control <br />structure) . <br /> <br />C. Overall map of the area showing where runoff presently does and size, location, <br />topography, and land use of off-site areas which drain through, onto, and from <br />the project. <br /> <br />D. Identification of seasonal water table elevations. If the project is in the known <br />flood-way of a natural stream, it should be identified and approximate flooding <br />depths determined. The 100 year flood plain elevations and limits should be <br />identified if applicable. <br /> <br /><:.;""",' <br /> <br />E. <br /> <br />Description of vegetative cover. Wetland areas and preservation or mitigation <br />proposals should be identified. <br /> <br />F. A recent aerial photograph of the project area with project boundaries delineated. <br /> <br />G. Paving, grading and drainage plans, with special attention to perimeter site <br />grading. <br /> <br />H. Percolation tests must be submitted if percolation or exfiltration systems are <br />proposed. Percolation tests shall be representative of design conditions. <br /> <br />I. Complete description of measures to be implemented during the construction <br />period to mitigate adverse quantity and quality impacts off-site. <br /> <br />J. Indicate whether either surface or groundwater withdrawals are proposed for <br />irrigation or other on-site water use. Because of the inseparable nature of Water <br />Use and Surface Water Management the application for either will not normally <br />be considered complete until all information for both is complete.
