Just complete the <br />application supplied by your EyeMed Sales Representative, and we'll send you an <br />implementation kit with all the information you need. <br /> <br />EyeMed will implement your plan once you've taken these easy steps: <br /> <br />Q Complete the application form <br />Q Submit member eligibility data or demographic data <br />Q Complete and execute the contract <br />Q Distribute member materials, including ill cards <br /> <br />Your EyeMed representative will gladly help you with any process-related questions you might <br />have. Your representative will also be happy to coordinate assistance at open enrollment <br />meetings where attendance will be 100 or more employees. <br /> <br />Integrated Administrative Systems <br />EyeMed's Administrative System's integrity is maintained through a comprehensive database <br />system that automatically updates all related files to include changes in member and provider <br />information. <br /> <br />To maintain the integrity of your organization's own vision care program, EyeMed requests <br />montWy updates of member eligibility.
