<br />DEFINITIONS <br />(continued) <br /> <br />..>AY OR DATE means at 12:01 A.M" at the Group Policyholder's primary place of business; when used with regard to eligibility <br />dates and effective dates. It means 12:00 midnight, at the same place, when used with regard to termination dates. <br /> <br />DENTIST means a licensed doctor of dentistry, operating within the scope of his or her license, in the state in which he or she is <br />licensed, A Dentist does not include a Covered Person or a relative of the Covered Person. Relatives include spouses, siblings, <br />parents, children and grandparents. <br /> <br />DEPENDENT: See the Eligibility for Dependent Dental Coverage section of this Policy. <br /> <br />DEPENDENT DENTAL COVERAGE means the coverage provided by this Policy for eligible Dependents, <br /> <br />ELIGIBILITY WAITING PERIOD means the continuous period of time that an Employee must be employed in an eligible class <br />with the Group Policyholder, before he or she becomes eligible to enroll for coverage under this Policy. <br /> <br />This Eligibility Waiting Period may be waived for an Employee who qualifIes for reinstatement of his or her coverage, as provided in <br />this Policy, <br /> <br />EMPLOYEE means a full-time employee of the Group Policyholder: <br />(1) whose employment with the Group Policyholder is the employee's principal occupation; <br />(2) who is not a temporary or seasonal employee; <br />(3) who is regularly scheduled to work at such occupation at least 30 hours each week; <br />(4) who is a member of an employee class which is eligible for coverage under this Policy; and <br />(5) who is a citizen of the United States or who legally works in the United States. <br /> <br />EMPLOYEE DENTAL COVERAGE means the coverage provided by this Policy for eligible Employees. <br /> <br />EXPENSES INCURRED (includes INCURRED). An expense is Incurred at the time a service is rendered or a supply is furnished, <br />except that an expense is considered Incurred: <br />(1) for an appliance (or change to an appliance), at the time the impression is made; <br />(2) for a crown or bridge, at the time the tooth or teeth are prepared; and <br />(3) for root canal therapy, at the time the pulp chamber is opened; <br />provided the service is completed within 31 days from the date it is begun. <br /> <br />GL 11-4.1-DF FL <br /> <br />5 <br /> <br />09/01/01 <br />
