<br />" \ <br /> <br />" <br /> <br />CITY OF SUNNY ISLES BEACH <br />ENTRANCE SIGNAGE CONTRACTOR AGREEMENT <br />CONTRACT NO. 03-0401-07 <br /> <br />HIS CONTRACTOR AGREEMENT made and entered into this / 7 ~ay of <br />2003, by and between the CITY OF SUNNY ISLES BEACH, FLORIDA, a <br />munlclp corporation of the State of Florida, (hereinafter referred as to "City") and <br />INTERNATIONAL SIGN AND DESIGN CORP., a Florida corporation, authorized to do <br />business in the State of Florida, (hereinafter referred to as "Contractor"), whose Federal I.D.# is <br />SS'~){3:s'1 q . <br /> <br /> <br />WHEREAS, International Sign and Design Corp., is a qualified, Florida contracting firm <br />experienced in the design and installation of commercial/industrial signs; and <br /> <br />WHEREAS, by Resolution No. 2003-51Q, the City qualified Contractor as the lowest <br />responsible bidder to City Bid No. 03-04-01 for entranceway signage to the City of Sunny Isles <br />Beach; and <br /> <br />WHEREAS, the City wishes to employ Contractor to design and install the entrance way <br />sign to the City as more specifically described in Exhibit "A," attached hereto and made a part <br />hereof; and, <br /> <br />WHEREAS, the Contractor is qualified, willing and able to provide the desired services <br />on the terms and conditions set forth herein; <br /> <br />NOW THERFORE, in consideration of the premises and the mutual covenants herein <br />named, the parties hereto agree as set forth below: <br /> <br />1. RECITALS. The Recitals set forth above are hereby incorporated into this Agreement <br />and made a part of hereof for reference. <br /> <br />2. SERVICES TO BE PERFORMED. Contractor shall perform the following services: <br /> <br />a. Specialized assistance in the design and installation of the entranceway sign for the City <br />of Sunny Isles Beach, as well as those services more particularly described in Exhibit <br />"A" attached hereto and made a part hereof. <br /> <br />b. Contractor will prepare and present oral/written reports as needed or as requested on a <br />periodic basis while engaged in special projects. Contractor shall provide, on a monthly <br />basis, to the City Manager, a detailed narrative report delineating all services provided by <br />the Contractor in the previous month. The Contractor shall also provide quarterly, <br />written reports, detailing all of the Contractor's activities in the previous quarter and the <br />results of the Contractor's efforts. Each report required herein will be submitted to the <br />attention of the City Manager. <br /> <br />c.
