<br />Eligibilitv Requirements for Applicants: <br /> <br />· Legally incorporated 501(c)(3) not-for-profit organization lacking access to <br />government sources of capital funding. <br />. Demonstrable financial stability. <br />. Organization's mission is consistent with goals identified in the Miami-Dade County <br />Strategic Plan. <br />. Demonstrate ownership of or intent to purchase a facility. <br />· Letter of Commitmcnt confirming the resources necessary to accomplish the project. <br />· Architectural/engineering study and/or equipment spccifications and professional <br />cost estimate. <br />· Two (2) year management and budget plan for the facility. <br />· Those listed in any Request For Proposal's related to this Fund. <br /> <br />c) Primary Hcalthcare Facilities Fund <br /> <br />The objective of this fund is to build and sustain the capability and capacity of the not- <br />for-profit sector and support entities that enhance the quality of primary healthcare <br />within Miami-Dade County by delivering needed services. The $25 million allocated to <br />this fund recognizes the importance and continuing contributions that these <br />organizations, and the care that they provide, make to the future of Miami-Dade County. <br /> <br />Eligibilitv Requirements for Applicants: <br /> <br />· Legally incorporated 50 I (c)(3) not-for-profit organization lacking access to <br />government sources of capital funding. <br />. ELIGIBILITY DETERMINATION AND EVALUATION <br /> <br />Following closure of an Application Submission Period, the County Manager will review and <br />evaluate each Funding Application Package for funding eligibility or ineligibility. The County <br />Manager may use entities such as the following to assist him in the review and to create Project <br />Review Committees. <br /> <br />Economic Dcvclopmcnt Fund <br />. Empowerment Zone Trust Board <br />. Miami-Dade Empowerment Trust <br />.
