<br />Assured Maintenance Agreement <br /> <br />Page 4 of 9 <br /> <br />Customer Name <br /> <br />City of Sunny Isles Beach <br /> <br />Agreement No. <br /> <br />Special Conditions: Repair service labor and emergency service labor will be billed at $85,00 per man per hour <br />for regular hours, $127,50 per man per hour for overtime and Saturdays, $170.00 per man per hour for Sunday <br />and Holidays. Service includes replacement of belts and pleated filters. Service does not include replacement of <br />(59) 12" 85% HEPA cartridge filters, Disposal of filters and belts will be in Sunny Isles City Hall's garbage <br />receptacles on site, Customer Is responsible for scheduling access to equipment with McQuay and <br />Owners/tenants. All service will be dispatched through the service office at (800) 487 -4808 <br /> <br />Duration <br /> <br />This Agreement shall remain in effect for an Initial term of 2 vear(s) beginning January 1,2007 <br />(the "Effective Date") and shall continue from year to year thereafter unless at least 30 days prior to the <br />expiration date of the Initial term or any extended term, either party gives the other written notice of its intention <br />to terminate this Agreement. <br /> <br />Price and Payment Terms <br /> <br />1. McQuay Factory Service will provide services pursuant to this Assured Maintenance Agreement for the sum of <br />$19,560.00 per annum. <br /> <br />2. Payment will be in advance as follows: on the first day of each month beginning on the Effective Date of <br />this Agreement, McQuay Factory Service will provide Customer with an invoice in the amount of $1,630.00 <br />and payment will be due upon receipt. <br /> <br />3, The first monthly payment will also include a one-time charge of. $ N/A <br />the Equipment in good operating condition prior to the first contract year, <br /> <br />to cover the cost of placing <br /> <br />4, The monthly payments will be decreased by $ N/A <br />during this period and which expires on: <br /> <br />due to the Equipment warranty which is in effect <br /> <br />5. Customer shall pay for all costs, including all taxes, fees and governmental assessments. in <br />excess of $ N/A per pound Incurred by McQuay Factory Service for refrigerant used In connection <br />with this <br /> <br />Agreement. <br /> <br />NOTE: This Agreement is subject to final approval by McQuay Factory Service. <br /> <br />\ .'
