<br />Channel Services Payment Plan Agreement <br /> <br />City of Sunny Isles Beach ("Subscriber") requests BellSouth Telecommunications, Inc. ("Company") to <br />provide the following Service as described below. The Service included in this Agreement is: <br /> <br />T1 -- Hicap <br /> <br />1. Service is provided pursuant to and in accordance with the Federal Communications Commission <br />Tariff No.1, including the Channel Services Payment Plan (CSPP). <br /> <br />2. The CSPP payment plan and service period for this service are: <br /> <br />D Plan A for Months <br />[gJ Plan B for 24 Months <br />D Plan C for Months <br />D Plan 1 for Months <br />D Plan 2 for Months <br /> <br />Recognition of previous service may be given to the Subscriber who renews an existing Arrangement <br />or converts from month-to-month billing, under circumstances detailed in the tariff. This Agreement <br />period includes 0 months for recognition of previous service. <br /> <br />3.
