C0708-054 <br /> <br />THIS FIRST ADDENDUM ("First Addendum") entered into this ~ day of ~, <br />2008, amends that certain Agreement between the City of Sunny Isles Beach, Florida ("City") and ALL <br />FLORIDA POOL & SPA CENTER, a Florida corporation, whose Federal Identification # is 59-1492794 <br />("Contractor"), under Contract No. C0607-105, with an effective date of September 17, 2007 (the <br />"Agreement") for fountain maintenance services at the City's Government Center. <br /> <br />For the consideration provided by this First Addendum, which are deemed by the Parties as <br />adequate consideration, the Parties agree as follows: <br /> <br />1. ADDITIONAL SCOPE OF WORK: The City wishes to add to the original Agreement to <br />construct a permanent liner for the fountain surrounding the Government Center Commission Chambers, <br />as more fully described in Attachment "A". <br /> <br />2. COST: The Contractor agrees to be compensated for the additional scope of work described <br />above, in accordance with the payments terms and conditions of the Agreement, an amount not to exceed <br />Eleven Thousand Seven Hundred Fifty Dollars ($11,750.00). <br /> <br />3. TERM: The term of this First Addendum shall begin upon the execution of this Addendum and <br />shall end on October 1,2008. <br /> <br />4.
