Last month, through a chance encounter I met
Mr. Beau Parillo, Executive Vice President of United Automobile Insurance Company ("UAIC"). Mr.
Parillo and I talked extensively about the City and the efforts/programs the City undertakes. After our
conversation, Mr. Parillo advised that UAIC would like to make a Five Thousand Dollar ($5,000.00)
donation to the City summer camp program. This donation will cover the costs for 7 12 children to attend
the City summer camp program for the entire summer (9 weeks). We have received the donation and
now ask the City Commission to sanction its acceptance.

ADDITIONAL INFORMATION:

UAIC, an insurance writer based in North Miami Beach, Florida, has a long history of philanthropic
endeavors. Some of their most recent include assisting the City of Miami Police Department install a Lo-
Jack tracking unit in a police helicopter; sponsoring the Mike Lowell's Celebrity Golf Classic to benefit
cancer patients and their families; teaming up with Miami Children's Hospital to provide one of the only
indoor Child Safety Seat Check stations in the country; and hosting a holiday party at Miami Children's
Hospital for sickle cell anemia patients.

Additionally, UAIC is a sponsor of Florida's Children First, a non-profit organization that represents the
interests of Florida's foster children, and the 2007 St. Jude Shoot Out, a charity golf tournament, dinner
and auction that provides funding for St. Jude Children's Research Hospital.

To be completed by Department Head To be comfJleted by City Clerk's Office
Funding available: Approval: Agenda Item No,: 10 t:J
Finance Department City Manager 5-15-0~
Commission Meeting Date:

Cover Memo $5,000 Donation To Summer Camp
