Schedule of Basic Bcnefits of the <br />Group Medical and Hospital Service Contract is amended as follows: <br /> <br />Each Member may select one Primary Care Physician (PCP) upon enrollment, but is not required to <br />do so. In the event that you do choose a PCP, the Health Plan must be notified and you must receive <br />approval prior to changing your PCP. Such change will become effective on the first day of the <br />month after you notify Health Plan, You cannot change your PCP selection more than once per <br />month. <br /> <br />I <br />1(.) <br />I <br />I <br />, <br /> <br />You are entitled to see participating Specialty Hcalth Care Physicians without a referral from your <br />PCP. Self-referral is not permitted to participating Specialty Health Care Physicians designated as <br />"Requires Special Consultation between your Doctor and the A vMed Medical Director" in the written <br />or electronic Provider Directories at the time of service. <br /> <br />Health Professionals may from time to time ceaSe their affiliation with Health Plan.
