Summary

City of Sunny Isles Beach has indicated in their Request for Proposal that they are interested in
identifying a broker/agent that is familiar with the South Florida markets to assit them with the
strategy and renewal process for their health, dental, life, ad&d and long term disability
insurances who has experience working with small to mid-size nonprofit organizations for the
following services:

. Providing counsel on benefit strategies

. We take great care to understand
the objectives of your benefits program before we make any recommendations on the design of
your plan. And once your plan is designed, we never stop reviewing your benefits to assure that
as your objectives, corporate structure and workforce change so too does your employee benefits
package.

The final plan design always stems from some combination of three criteria: your budget, your
objectives and your employee's needs. Depending on the budget and objectives, some

4
