In <br />regards to the CSBE requirement, are there any special forms that must be <br />submitted with the bid, ex Schedule to Intent Affidavit. If so, please <br />forward the form/forms." <br /> <br />Certified Small Business Enterprises (CSBE) is not a requirement, but it <br />is recommended and encourage from Prospect Bidders. <br /> <br />Question: <br /> <br />Answer: <br /> <br />"Has the City conducted tests to determine casing depth for the wells as <br />required by Water Management District for the well permit. If not I <br />assume the drilling company will be required to obtain the permit as <br />usual. " <br /> <br />Permit from DERM was secured by the City. Contractor must pay and <br />obtain the permit with DERM (payment will be reimbursed by the City). <br /> <br />Question: <br /> <br />Answer: <br /> <br />"Sheet # 36 states the well must be able to accommodate 600 GPM. Well <br />performance is usually specified as GPM per foot of head or specific <br />capacity. Is this correct or was "per foot of head" left off'?" <br /> <br />It must read "per foot of head" <br />
