<br />(e) That the sponsoring agency of the community residential home <br /> <br />notify the Director in writing of its intention to establish said <br /> <br />facility. Such notice shall contain the address and legal description <br /> <br />of the site, the number of resident clients, as well as a statement <br /> <br /> <br />from the State of Florida Department of Health and Rehabilitative <br /> <br /> <br />Services indicating the need for and licensing status of the <br /> <br /> <br />proposed facility.
